PTO Funding Request Forms
The Barnum PTO is able to help fund special requests due to the support from Barnum parents, school staff, and our community. Please understand that the PTO must weigh the pros and cons of each request in order to be fair to all groups.
Download the appropriate form below, fill it out, and then send email to barnumpto@gmail.com. Your request will be reviewed at the next PTO meeting. Please make sure a representative is available at the next meeting to answer any questions.

Groups are able to request up to $500 for special requests. Special Funding Request Form
- Field Trip Funding Request Form
Groups are able to request up to $500 to cover field trip transportation expenses. If transportation expenses are covered through scholarships or grants then your group can request up to $2/person to cover other field trip expenses. Field Trip Request Form
