Form Requirements for Athletic Participation

1. MSHSL Eligibility Brochure read, filled out and signed by student and parent each year
2. BHS Activities Handbook read and signed by student and parent each year
3. MSHSL Sports Physical by a doctor once every three years clearing student for participation. The form must be signed and dated by a doctor and be on file at the school.

Forms are due in the high school office BEFORE the first practice of the season in which the student is participating. These forms must be filled out each school year (except sports physical), but only one time per year is necessary even if the student is participating in more than one sport.

Barnum High School is a member of the Minnesota State High School League (MSHSL).
